For your patient Most people living with a life-limiting condition would prefer to stay living in their own home. For this reason over 80% of our care is delivered in the community.
Woking & Sam Beare Hospice is committed to providing care that is tailored to the individual at the time they need it but also in the place of their choosing. We know that most of our patients prefer to be cared for in their own home, whether this be a private residence or in a care or nursing home, and so our focus is on working with you, our healthcare colleagues, to ensure the wishes of patients and their carers and to avoid unnecessary hospital admissions.
Our care is delivered by our community teams, on the ward in the hospice or patients can access a range of services through our Bradbury Wellbeing Centre.
