Accessing our services

In order to access our services you will need to be referred to our Clinicl Team via your General Practitioner [GP], Community Team, District Nurse or hospital Palliative Care Team.

Patients are generally visited at home, care home or in hospital prior to acceptance.

All referrals for admissions are assessed daily, Monday to Friday. Emergency referrals will be assessed and a decision on whether admission is appropriate will be decided and prioritised.

While the work of the hospice is not exclusive, our patient referral region covers six Surrey Boroughs [Spelthorne, Runnymede, West Elmbridge, Surrey Heath, Woking, North Guildford].

However, we do also provide support to people from outside of that area when required and your healthcare professional who makes your referral can advice you about this.
